Heading Home, also known as The Heading Home Initiative, follows a collective social impact model which relies on the strength of its multi-agency partnership. 

This collaboration brings about measurable and lasting change through our community’s efforts to end chronic homelessness.  Each agency’s strengths and expertise combine together to create permanent housing solutions for our most vulnerable neighbors. Heading Home is truly a city-wide collaboration that includes many public, private, government, and non-profit partners.

Heading Home is about solutions. Our passion for our work is fueled by a desire to make important, positive and lasting change. We care about the people we serve!

We aim to make experiences of homelessness rare, short-lived, and non-recurring. We achieve this by providing permanent, supportive housing solutions to people and their families who are medically vulnerable and have been experiencing chronic homelessness.
